As mandated by Republic Act no. 3753, or the Civil Registry Law, the birth, marriage, and deaths of Filipinos should be reported to and registered with the Philippine Statistics Authority (PSA) through the Embassy having jurisdiction over the locality where the event took place.
Please be advised that Civil Registry services are scheduled on Tuesdays and Thursdays.

REPORT OF BIRTH - GENERAL REQUIREMENTS:
- All documents not in English must be accompanied by its translated version
- Four (4) duly-accomplished and originally-signed Report of Birth Form (download here).
If applying in-person: Applicants must sign the ROB Forms at the Philippine Embassy before a Consular personnel. Civil Registry services is available on Tuesdays and Thursdays.
If applying by mail: Four (4) ROB Forms must be originally-signed and duly notarized by a Notary Public in Sweden or in Finland.
- Original and three (3) photocopies of the following documents:
- Child’s foreign birth certificate. If the document is not in English, it must be accompanied by its translated version
- Parent’s Philippine Statistics Authority (PSA)-issued Marriage Certificate or Report of Marriage (ROB) issued by the Embassy
- PSA Birth Certificate of Filipino parent and foreign birth certificate of foreign parent
- Photocopies of the passport data pages of the parents
- Proof that the parent/s was/were Filipino/s at the time of birth of the child (examples: visa, identification certificate, naturalization certificate, etc.). If the parent/s is/are dual citizens, submit a copy of the foreign naturalization certificate and Identification Certificate
- Processing fee of 350 SEK to be paid via Swish (for applicants in Sweden) or bank transfer (for applicants in Finland)
- For applicants who wish to receive back their documents by mail: Self-addressed, self-stamped, envelope
- The Embassy reserves the right to require additional documents from an applicant, to prove his/her identity and/or citizenship, and to ensure accurate and complete personal data entries, pursuant to the Foreign Service Act (R.A. 7157)
If a Report is filed more than twelve (12) months after the child’s birth:
- One (1) originally-signed Affidavit of Delayed Registration of Birth (download here)
- Additional Legalization fee of 350 SEK per document
If the parents had got married after the birth of the child:
- One (1) originally-signed Joint Affidavit of Legitimation duly accomplished by the parents (download here). Additional Legalization fee of 350 SEK per document
If the parents are not legally married and the father’s surname will be used by the child:
- One (1) originally-signed Affidavit of Admission of Paternity (AAP) accomplished by the child’s father (download here)
- One (1) originally-signed Affidavit to Use Surname of the Father (AUSF). One (1) originally-signed Sworn Attestation (SA) (download AUSF here and here) accomplished by the child’s mother
- Certificate of Registration of the above Affidavits will be issued by the Embassy
- Additional Legalization fee of 350 SEK per document

REPORT OF MARRIAGE – GENERAL REQUIREMENTS:
- All documents not in English must be accompanied by its translated version
- Four (4) duly-accomplished and originally signed Report of Marriage Form (download here)

- Birth Certificates of husband and wife. For Filipino applicants, Birth Certificate must be issued by the Philippine Statistics Authority
- Proof that the spouse/s was/were Filipino/s at the time of marriage (e.g.: visa, identification certificate, naturalization certificate, etc.). If the spouse/s is/are dual citizens, submit a copy of the foreign naturalization certificate and Identification Certificate issued by the Embassy.
- For annulled Filipino spouse: Philippine Statistics Authority (PSA) Marriage Certificate with annotation if marriage is annulled
- For divorced foreign spouse: Divorce decree
- For widowed spouse: Philippine Statistics Authority (PSA) Death Certificate or Report of Death issued by the Embassy or Foreign Death Certificate in English of the deceased spouse
- For applicants who wish to receive their documents back by mail: Self-addressed, self-stamped, envelope
- Processing fee of 350 SEK to be paid via Swish (for applicants in Sweden) or bank transfer (for applicants in Finland)

REPORT OF DEATH - GENERAL REQUIREMENTS:
- All documents not in English must be accompanied by its translated version
- Four (4) duly-accomplished Report of Death Form (download here) or Report of Fetal Death Form (download here) originally-signed by the informant
- One (1) original and four (4) photocopies of the Foreign Death Certificate of the deceased. All documents not in English must be accompanied with a translated version.
- The Death Certificate must contain the following information about the deceased certified by the attending physician: name, age, sex, nationality, date and place of birth, date and place of death, and cause of death.
- Four (4) photocopies of the passport data page, reflecting the photo and the complete name, of the deceased
- Proof that the deceased, at the time of death, was Filipino (examples: visa, identification certificate, naturalization certificate, etc.)
- If the Report of Death is filed more than one (1) year after the demise of the deceased:
- One (1) originally signed and three (3) photocopies of the Affidavit of Delayed Registration of Death (download here)
- Processing fee of 350 SEK to be paid via Swish (for applicants in Sweden) or bank transfer (for applicants in Finland)
- For applicants who wish to receive back their documents by mail: Self-addressed, self-stamped, envelope
FOR SHIPMENT OF HUMAN REMAINS TO THE PHILIPPINES:
Photocopy of the following documents:
- Coroner’s Certificate of the Fact of Death, or in its absence, an interim Death Certificate issued by the Coroner’s Office
- Embalming Certificate issued by the funeral parlor
- Certificate of Sealing of Casket
- Certificate of Non-Contagious Disease issued by the local health authority that contains the following information:
- Statement that the deceased did not die of a contagious disease; or
- That the remains to be shipped did not originate from a place contaminated by a contagious disease
- Removal Certificate issued by the local authorities
- Contact details, including address, and the flight details of the person who will bring the remains to the Philippines
- Passport of the deceased
- Consignee’s complete contact details
- Consular Mortuary Certificate, to be issued by Embassy after submission of complete requirements, with a Legalization fee of SEK 350 per document
FOR SHIPMENT OF CREMATED REMAINS TO THE PHILIPPINES:
Photocopy of the following documents:
- Coroner’s Certificate of the Fact of Death, or in its absence, an interim Death Certificate issued by the Coroner’s Office
- Certificate of Cremation issued by the Crematory
- Certificate of Non-Contagious Disease issued by the local health authority that contains the following information:
- Statement that the deceased did not die of a contagious disease; or

- Passport of the deceased
- Consignee’s complete contact details

If the remains are not of a Filipino national, then the requirements for Report of Death are not needed.
